Change of Address - Community Care

Permanent Change of Address

Community Care is available if the customer is unable to live on their own without support.
The customer can receive community care whether or not they live alone - eligibility for services is related to their care needs.
This may include services ranging from meals-on-wheels and/or a home help, to admission into a residential/nursing home.
The local Health and Social Services Community Trusts are responsible for assessing what people's care needs are, and for making the necessary arrangements to meet those needs, e.g. in a residential care or nursing home.

Temporary Change of Address

Respite Care

Respite care is when the customer and/or their partner are admitted to either a residential or nursing home, because they need to be looked after for a short period of time, while their normal carer is unable to look after them, or in order to give their carer a break.
Community Care is available if they are unable to live on their own without support.
The customer can receive community care whether or not they live alone - eligibility for services is related to their care needs.
This may include services ranging from meals-on-wheels and/or a home help, to respite care in a residential home or nursing home.
The local Health and Social Services Community Trusts are responsible for assessing what people's care needs are, and for making the necessary arrangements to provide the services required to meet those needs (including the needs of a carer where appropriate), e.g. in a residential care or nursing home.
